JOB DESCRIPTIONPATH CASE MANAGER- SUPPORTED HOUSING
DEPARTMENTDESCRIPTIONThe PATH Case Manager will provide assistance
in obtaining and coordinating services for eligible homeless
individuals. The PATH Case Manager will provide community outreach,
internal/external staff training, and case management services in
the ten county service area. The PATH case manager is also
responsible for marketing the permanent housing Program(s),
documenting case notes and program activities, entering data and
prepare monthly reports reflecting such activity, as indicated. The
PATH Case Manager will also ensure data and program compliancy with
all state and federal grant guidelines.QUALIFICATIONS---
Undergraduate degree in social work preferred or behavioral
science--- Minimum of 1 year successful experience in adult case
management--- One Year of Housing experience preferred.--- Ability
to work with homeless individuals of diverse backgrounds and
disabilitiesincluding physical, mental, substance abuse, HIV/AIDS,
etc--- Must have fundamental knowledge of Microsoft Office
programs.--- Outstanding organizational, verbal and written
communication skills--- Ability to perform daily duties to achieve
excepted outcomes and performancemeasures with minimal
supervision--- Ability to travel to various sites daily in the
ten-county region.--- Applicant must have a valid driver's license
and reliable transportation with current
insurance.RESPONSIBLITIES--- Provide case management to eligible
PATH clients. Case management will involve,screening, intake,
assessment, service plan development,monitoring, linkage to
appropriate community resources, follow up, appropriatedischarge,
tracking, etc.--- Assist participants with completing lease
agreements and obtaining other supportive documents such as
identification, social security card, etc as required by leasing
agents/property;--- Maintain up to date information regarding
tenant occupancy, move outs, and vacancies;--- Facilitate and
coordinate supportive activities with other program staff
including:employment assistance activities, assisting clients with
obtaining income through SSI/SSDI referrals or application
assistance; referring clients to mental health and medical
services; providing transportation resources; etc.;--- Maintain
thorough and complete record of participant's activity/progress;---
Establish and maintain collaborative working relationships with
homeless sheltersand community resources;--- Prepare and submit
reports to supervisor on a timely manner basis;--- Enter
participant's information into the HMIS database in a timely matter
and update asneeded;--- Attend appropriate coalition and other
community resource meetings;--- Attend training and workshops as
they come available,--- Develop promotional strategies to inform
the community about the program,including community presentations,
print material, and related activities;--- Must be able to work a
flexible schedule;--- Perform other assigned duties in a timely and
efficient matter. All 30+ hour positions will be enrolled in our
company retirement program through the Kentucky Public Pension
Authority (KPPA); six percent (6%) of your gross income will be
deducted from each check. All LifeSkills Inc. / Pennyroyal Inc.
employees who work 30 hours or more a week are required to
participate in this program.The employee contribution rate is set
by the State General Assembly. Employee contributions are deposited
into member accounts. These contributions to the retirement system
are currently tax deferred. Member accounts are kept confidential
between the member and the Kentucky Retirement Systems.
