Health Nurse Practitioner to join our team at the Community Based
Outpatient Clinic (CBOC) in Bowling Green KY. In exchange for your

VA CBOCs in the United States as a contractor for the U.S.
Department of Veterans Affairs. We provide a full range of medical
services to veterans through the operations of CBOCs, tailored to
meet the specific needs of local VA medical centers. Our
comprehensive set of services includes primary care, diagnostics,
laboratory, telehealth, behavioral health, and more.As a Mental
Health Nurse Practitioner, you will provide prescribed medical
treatment and personal care services to patients with diseases and
injuries seeking treatment in the clinic, as directed by physician
or mid-level provider. ?You will collaborate with the core PACT
Team (Primary Care Provider, RN and Medical Assistant) and expanded
PACT Team including family/caregiver, VA, and community-based
services involved in developing the patient care plan.?Core
ResponsibilitiesEvaluates veterans for mental health disorders and
appropriately diagnoses and treats mental health conditions.Orders
and performs diagnostic tests and therapeutic procedures as related
to mental health diagnoses and treatment and interprets test
results for deviations from normal and provides follow up
care.Prescribes medication and recommends dietary and activity
programs.Performs appropriate interval follow up visits for
necessary treatment of mental health diagnosis.Refers patient to
the VA medical specialist when indicated. Counsel patients on the
use of prescription medications and educates patients in relation
to treatment plan.Develops and implements patient management plans,
assists in provision of continuity of care through the PCMM
(Primary Care Management Module).Evaluates patients records from
medical providers outside the VA and works with the veterans
utilizing rules set forth by the VA for co-managed care.Conducts
individual counseling/therapy.Contributes to group therapy (i.e.,
PTSD groups; weight management; smoking cessation; grieving; and
personal interactions).Contributes to pain management plan as
needed.Involves veterans in planning and assuming responsibility
for activities.Refers veterans, when indicated, to community
resources and other organizations.Addresses issues such as advance
directives and organ donation in accordance with acceptable
standards.Inputs all patients information into VISTA/CPRS.Assists
in co-managed care collaboration with non-VA providers, and
facilities.Maintains confidentiality of veterans data and
information.Complies with all VA and company training
requirements.Fulfill compliance requirements of the Office of
Inspector General (OIG), Joint Commission(JC), Environment of Care
(EOC) oversight, lab compliance and other related items.Remain
focused on achieving excellent clinical outcomes through the
specified VA guidelines.Must provide excellent customer service to
each veteran and his/her family, both in person and over the phone,
as well as to fellow colleagues and clinic visitors.Participate in
the clinics outreach events to help support the clinics enrollment
initiatives.Support patient enrollment and retention in
appointments, follow-up visits or nurse visits.Embrace and support
new initiatives, whether clinical or
operationalRequirements:QualificationsGraduate of an accredited
school of nursing and graduate of an accredited program for nurse
practitioner (MSN required), including preceptorshipCertification
by applicable professional organizationMinimum three-year clinical
experience as an adult mental health CRNP. (Specific requirements
could vary based on individual VA contract).Must be credentialed
and remain in good standing through the Veterans Health
Administration (VA).Must comply with and maintain all requirements
for a valid, unrestricted license in the state of desired
employment, or in any U.S. state or territory, depending on VA
contractual requirements.Current certification in Basic Life
Support (must be renewed periodically as specified by the
certifying agency AHA valid for two years, e.g.) and in ACLS as
specified by individual VAMC contractCurrent, unrestricted Drug
Enforcement Administration (DEA) registrationProficiency in written
and spoken English.Strong computer skillsEnergetic and optimistic
demeanorStrong service mentality and a focus on achieving all
aspects of defined service standardsThis is considered a safety
