Practitioner, Adult Nurse Practitioner, Geriatric Nurse
Practitioner, Physician Assistant Start date: ASAP - Ongoing Shifts
requested: Monday - Friday, 8:00 AM - 6:00 PM Practice setting:
Home Health Patient load: 5 to 8 assessments per day (assessments
last approximately 45-75 minutes) Ages: Geriatrics Procedures
required: In-home health and wellness assessments for health
insurance plans, H&P evaluations, review medications/dosages,
physical exams, blood pressure checks, finger sticks, etc.
Requirements: CAQH, NPI number, state NP/PA license, auto insurance
($100K coverage), unrestricted state license, Medicare/Medicaid
enrollment (cannot have opted out), required personal medical
equipment (lab jacket, stethoscope, ophthalmoscope, monofilament,
tuning fork, reflex hammer, blood pressure cuff, pen, portable
scale, tape measure, pulse oximeter) Other: Providers must be
willing to travel at least 75 miles from home, client will not
accept anyone who travels less than 75 miles. Minimum availability:
3 consecutive days per week every week or 2 full weeks per month.
Credentialing timeframe: 30-45 days. EMR: Proprietary system on
iPad (provided by client). PPE provided by client. Mileage
reimbursement for 76 miles from home; rental car and hotel
available based on shift commitment. Benefits: Malpractice
insurance coverage; weekly electronic pay; potential travel and
