Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 Experience: Minimum 1 year License: RN License Certifications: BLS Must-Have: 1. Experience in case management. 2. Strong communication and advocacy skills. Description: Coordinate the overall interdisciplinary plan of care for patients from admission to discharge. Create a plan for care across the continuum, integrating patient and family preferences and values. Monitor patient medical necessity and level of care through assessments, ongoing evaluations, and patient records. Advocate for resources and the removal of barriers. Maintain ongoing dialogue with supervisor and care transition team members to ensure effective implementation and reevaluation of health plans.
