Job Description:
HCA Healthcare is seeking a dedicated and compassionate Home
Health RN to join our expansive team of healthcare professionals.
In this role, you will provide direct patient care within the
patient's home, implementing and documenting nursing interventions
and coordinating overall nursing care for patients based on their
individual health needs. With HCA Healthcares resources and
support, our nurses have a unique opportunity to be advocates of
patient care while maintaining autonomy and flexibility in their
day-to-day work. Assess patient conditions during visits and carry
out the appropriate care plans. Educate patients and their families
on proper home health care strategies and procedures. Coordinate
with physicians and other healthcare professionals to create
customized care plans for each patient. Monitor patient recovery
and compile reports for healthcare team making necessary
adjustments to care plans. Ensure adherence to all health and
safety standards and procedures. Support patient independence and
treat them with dignity and respect. Strong clinical skills in a
home health setting Excellent interpersonal and communication
skills Proficient in documenting and maintaining patient records
electronically Able to handle emergency situations calmly and
efficiently Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Current Registered Nurse (RN) license in the
state of practice Basic Life Support (BLS) certification Driver's
license and reliable transportation
