Job Description:
As a Home Health RN Clinical Manager at HCA Healthcare, you will
oversee the provision of home-based patient care in accordance with
our values of quality, compassion, and effectiveness. You'll lead a
team of skilled nurses and support staff to ensure exceptional
service delivery, focusing on patient outcomes and adherence to
compliance standards. This important role is pivotal in shaping our
community's health directly from the comfort of our patients'
homes. Manage day-to-day clinical operations of the home health
care team, ensuring compliance with applicable laws, regulations,
and company policies. Oversee patient care planning and ensure the
implementation of clinical practices are in line with the latest
evidence-based guidelines. Conduct regular staff meetings and
training sessions to ensure nursing practices meet the high
standards set by HCA Healthcare. Monitor patient care processes and
outcomes, utilizing data to drive improvements in service delivery.
Liaise with other healthcare providers, departments, and community
organizations to facilitate integrated patient care. Manage budget,
staffing, and resource allocation effectively, maintaining
operational efficiency. Strong leadership and organizational skills
Excellent communication and interpersonal skills Proficiency in
clinical decision-making and problem-solving Knowledge of home
healthcare laws and regulations Ability to manage multiple
priorities in a high-paced environment Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current RN license
in the state of practice CPR certification Preferably certified in
home health as a CHHCM (Certified Home Health Case Manager) or
relevant specialty
