Come join our island of misfit toys!Placement/Intake/AssessmentEnsure initial legal documentation is obtained prior to placement or service deliveryEnsure all information required by regulation is obtained according to program guidelinesCollaborate with referring agency to address need areas and ensure the case plan being followedDefine boundaries for immediate intervention to ensure safety, permanency, and well-beingEnsure that assessments and treatment plans are completedOngoing Treatment/DischargeProvide oversight to meet the provision of structure in activities of client and/or caregiver to ensure safety of client, caregiver, and/or communityMeet and/or exceed all state and licensure regulations regarding face-to-face and individualized contact with the clients on your caseloadServe as liaison betweenNeccoand referring agency and all necessary community resource entitiesEnsure compliant, accurate and timely completion of assessments and treatment plansProvide and/or assist with transportation and/or visitation as neededCollaborate with all vested external service providers as needed to ensure continuity of services forthe client and/or familyMonitor, document and review program specific areas of concern as well as progress in those areasMaintain confidentiality as outlined in policyCrisis Prevention/ManagementEnsure proper documentation, reporting, and staffing ofall critical incidentsCollaborate with supervisor and team regarding any critical incidentsReport to supervisor and/or designated entities any instances of non-compliance withNecco, regulatory rules/policies, reports of suspected abuse, neglect, or any incident defined as "critical"Data EntryEnsure complete and accurate data is entered into the Electronic Health Record, according to timeframes outlined in policy and/or regulationTeam ResponsibilitiesAttend and participate in self-directed dailyhuddles/weekly tactical/monthly strategic meetings and activitiesPerform other duties and responsibilities as assigned by immediate supervisorParticipate in achieving our mission: We Build FamiliesParticipate in the performance quality improvement process and execute any assigned tasks related to the processEnsure that all employee-related documents are current and up to date for your employee personnel fileCorporate CitizenPractice ruthless pragmatismEngage in peer to peer feedbackKnow and live theNeccoCorporate Culture PrinciplesEmbody the three essential virtues of humble, hungry and smartDrive your Individual Performance ScorecardAdhere to and contribute to theNeccomeeting structurePosition Qualifications:21 years of ageCurrent LSW or LPC or Bachelor's Degree in a human services-related field and a minimum of 2years experienceworking with clients with emotional or behavioral concernsValid Driver's LicenseLimited Liability Auto Insurance Coverage of 100/300/100Organizational, and Written/Oral Communications skillsMust have the skills necessary to navigate and execute several software's (i.e. Microsoft offices, Databases,Electronic Health Records, Dashboards, etc.)Successful completion of all required Criminal Background checks and drug screenings per state and company regulationsAbility to adhere to scheduled and unplanned deadlinesWillingness to travel and attend trainingSuccessful completion of all required training pertaining to job Equal Opportunity Employer/Protected Veterans/Individuals with Disabilities The contractor will not discharge or in any other manner discriminate against employees or applicants because they have inquired about, discussed, or disclosed their own pay or the pay of another employee or applicant.
